The breaking point came from watching patients cycle through the same revolving door, knowing that with the right infrastructure, coordination, and follow-up systems, those readmissions were entirely preventable.

Our leadership team includes former hospital directors of care coordination and case management, staff psychiatrists with ED experience, and clinical leaders who have managed behavioral health programs at scale.
